Dear practitioners, therapists, volunteers and supporters of the Arizona Oncology Foundation,

As an organization whose focus is entirely on caring for those in the Southern Arizona cancer community, we find ourselves on the front lines of the global outbreak of COVID-19 (coronavirus). The virus has officially been declared a pandemic by the World Health Organization and a public health emergency by Arizona Governor Doug Ducey. Together, we will do
everything in our power to protect the health of our communities.

At this time our duty is to help stop the spread of the virus to avoid community contagion, particularly among those who could be most vulnerable.

We have made the decision to consolidate our resources, volunteers and personnel, and to operate only from our Craycroft Resource Center, located at 2625 N. Craycroft Rd. Ste. 215, until Tuesday, March 31st. Ample signage at our Rudasil Resource Center informs patients, clients and other visitors of this change.

We will have a staff member at the Craycroft Resource Center Monday through Friday from 9:00am – 5:00pm to provide assistance to our patients.

Volunteer support will be restricted throughout this time period. We will reassess the situation at the end of March and will provide further updates at that time. Until then, our Program Manager, Ron Bridgemon and available volunteers, will be happy to help reschedule/cancel any appointments or services.

These changes have been implemented to prioritize the absolute safety of our staff, volunteers, practitioners and the community we serve.

Gas Cards & Bus Passes

Gas Card Eligibility Requirements:

  • Currently undergoing treatment.
  • Traveling 20 miles or more one way for treatment.
  • Must meet financial hardship guidelines of oncology practice.

Bus Pass Eligibility Requirements:

  • Currently undergoing treatment.
  • Must meet financial hardship guidelines of oncology practice.

Application Precess:

  • Obtain Transportation Assistance Referral Form, from Arizona Oncology Foundation Resource Center or click HERE to download.
  • Patient’s physician must fill out and sign the top portion certifying that patient meets financial need criteria.
  • Patient must fill out bottom demographic portion.

Lodging

Our Northern Arizona locations have several programs and services to help cancer patients with what they need. Major services include gas cards and bus passes for transportation to and from treatment, as well as lodging (hotel) vouchers during treatment and educational opportunities. House Cleaning Program

House Cleaning Program

We focus on your home, so you can focus on YOU!

Complimentary house cleaning for patients. If you are in active treatment, let us help you focus on healing and not on housework. We partner with local professional house cleaning service companies that are fully licensed and bonded.

Qualifications:

  • Must be in active, infusion-based treatment.
  • Qualified patients may be approved for up to one service per month while funds last.

How to Apply:

  • Please complete referral form and have it signed by your oncologist or oncology representative. Tucson Area Form | Phoenix Area Form
  • Return completed form by email to Coordinator@ArizonaOncologyFoundation.org or drop it off any AOF Resource Center.
  • Once approved, we order services for you and the housecleaning partner will contact you to make the appropriate arrangements/appointments.
